About the Role: The Supportive Living Coordinator is responsible for the delivery of comprehensive, recovery-oriented case management for individuals living with severe and persistent mental illness (SPMI). Operating within a psychosocial rehabilitation framework, you will facilitate housing retention and community integration for a caseload of clients with complex needs. This role serves as the critical link between clinical care and independent living, requiring a balance of compassionate support, advocacy, and rigorous tenancy management.
This is the minimum pay of $17 per hour. The location is located in Iqaluit
Key Responsibilities:

Clinical Case Management & Planning:

Develop, implement, and review comprehensive Individualized Service Plans (ISPs) in collaboration with clients and their clinical teams.

Conduct regular acuity assessments to monitor mental health stability and adjust support levels as required.

Utilize a strength-based approach to support clients in achieving personal recovery goals related to education, employment, and social connection.

Housing Retention & Tenancy Support:
-
Act as a liaison between landlords and tenants to mitigate eviction risks and resolve tenancy disputes under the Residential Tenancies Act.

Conduct regular home visits to ensure safety, hygiene standards, and property maintenance are upheld.

Provide hands-on coaching in life skills essential for independent living, including financial literacy/budgeting, meal planning, and medication management.

System Navigation & Advocacy:

Navigate complex social systems to secure income support (e.g., AISH, welfare), legal aid, and primary health care for clients.

Facilitate referrals to specialized community resources such as substance use treatment, food security programs, and vocational training.

Crisis Intervention & Safety:
-
Identify early warning signs of decompensation or relapse and intervene proactively to prevent hospitalization.

Execute crisis protocols when necessary, including suicide risk assessments and coordination with mobile crisis teams or law enforcement.
Qualifications & Competencies:
-
Education:
-
Bachelor’s Degree in Social Work (BSW), Psychology, Nursing, or a related discipline.

Registration with a professional regulatory body (e.g., ACSW, CACP) is strongly preferred.

Experience:
-
3+ years of experience in community mental health, supportive housing, or homeless outreach.

Proven experience working with high-acuity populations, including those with concurrent disorders (mental health and substance use).

Technical Skills:

In-depth knowledge of the Residential Tenancies Act and Mental Health Act.

Proficiency with case management software (e.g., HMIS, HIFIS) and strong documentation skills for legal/clinical compliance.

Valid Driver’s License and access to a reliable vehicle (Mandatory for community visits).
